Why does Windows 10 show a "SmartScreen warning"?

We have been asked why Windows 10 something throws a "SmartScreen" warning. This has to do with our application's code signing certificate, and how complicated Microsoft has made the signing process. This error typically doesn't happen, or at least it doesn't happen everytime and on every machine, so we have found it difficult to get Microsoft to replicate and provide a fix. If you were to download the application several (5+) times and re-install it each time, the SmartScreen error would disappear. Windows simply shows this warning because it's a new .exe file that it has never esen before. We're looking to get this resolved and updated in a future update.
